Pre-Paid Dental Plan
Welcome to Our Dental Plan
This is a pre-paid dental plan available to our patients to be used at our practice.
Pre-Paid Dental Plan Eligibility
Anyone not covered by dental insurance is eligible. You can register as an adult or a child, though patients 18 years and older are considered adults and must apply as such.
Annual Costs:
- Adults - $300.00
- Child - $250.00
**If you need a periodontal cleaning, only the cost of the regular cleaning will be deducted as part of the plan. **
Pre-Paid Dental Plan Benefits
- Two regular cleanings
- A comprehensive exam and a periodic exam or two periodic exams
- Bitewing x-rays
- One Fluoride treatment – Child Member
- One Panoramic x-ray – Adult Member
- 20% discount on all other services except:
- Treatment by dentist or staff outside of the practice.
- A general anesthesia as well as hospitalization for dental procedures.
- Orthodontics and dental sleep medicine.
